We are looking for a caring, motivated, and relationship-focused Wraparound Facilitator to support children and families with complex needs in Kanawha County and surrounding areas. In this role, you will work side by side with families, helping them identify goals, build strong support teams, and connect with the services and resources they need to create lasting stability and success.

What You'll Do

As a Wraparound Facilitator, you will play an important role in helping families feel supported and connected. Your work will include:

  • Partnering with children and families to guide them through the wraparound process
  • Facilitating team meetings that bring families, providers, and natural supports together
  • Creating individualized, strengths-based plans that reflect each family's goals and needs
  • Connecting families with community resources, school supports, mental health services, and other helpful programs
  • Monitoring progress and adjusting plans when needs or circumstances change
  • Working collaboratively with therapists, case managers, educators, and other service providers
  • Supporting families in building coping, communication, and problem-solving skills
  • Advocating for the family's voice, choice, and active involvement throughout the process
  • Keeping clear, timely, and accurate documentation
  • Using a trauma-informed and culturally responsive approach in every interaction
What We're Looking For

We're seeking someone who is compassionate, organized, and committed to helping others succeed. Ideal candidates will have:
  • A bachelor's degree in social work, psychology, human services, or a related field
  • Experience working with children and families in social services, behavioral health, or community-based settings preferred
  • Knowledge of the wraparound model and systems of care preferred
  • Strong communication and relationship-building skills
  • The ability to manage multiple responsibilities while staying organized and responsive
  • Confidence working independently and as part of a team
  • A commitment to confidentiality, professionalism, and ethical practice
  • Experience with case management or service coordination is a plus
