This person will learn and understand available community resources and programs; utilize data and local knowledge to build grassroots, strategic, and data informed community partnerships; and identify opportunities to develop and pilot new S - Do. H (Social Determinants of Health) initiatives. Job Duties. Implement and scale the overall strategy to address Health Equity & S - Do. H for members and communities within assigned region. Operationalize S - Do. H product within assigned region. Build partnerships to incorporate Health Equity solutions into S - Do. H, population health, care management, quality, regulatory, and community outreach programs. Establish outcomes-based contracts with community-based organizations. Organize community activation meetings to successfully execute key Health Equity & S - Do. H initiatives with community partners.
