We are seeking dedicated Nurse Practitioners (NPs) and Physician Assistants (PAs) for a rewarding PRN opportunity focused on conducting in-home Health Risk Assessments (HRAs) for Medicare Advantage members. This position allows you to enhance your income while working independently and positively impacting patient care, all without the burdens of ongoing treatment obligations.

Your Responsibilities:

• Conduct thorough in-home HRAs for Medicare Advantage patients.
• Review patients' medical histories, current medications, lifestyle, and significant social factors.
• Perform basic physical and mental health evaluations.
• Provide preventive care education—no ongoing patient management is required.
• Travel locally to assess patients within your county and surrounding areas.

Requirements:

• Active NP or PA license (Pediatrics, PMHNP, and Women's Health specialties are excluded).
• Availability to work at least eight (8) full 7.5-hour days per month.
• Willingness to travel within approximately 60 minutes of your residence.

Additional Information:

• As a 1099 contractor, you may deduct mileage at $0.725 per mile in 2026 (consult a tax advisor for details).
