Summary

The Advanced Practice Provider, either a Nurse Practitioner or Physician Assistant, ("Advanced Practice Provider") will function as independent healthcare provider who, working collaboratively within a multidisciplinary team and at the direction of the Outpatient Clinical Director and rural health clinic ("RHC") Medical Director, is responsible for providing comprehensive care to patients with acute and chronic medical conditions in an outpatient setting. The Advanced Practice Provider will primarily work at the Boone Memorial Hospital, Inc. ("BMH") Family Medical Center Walk-In Clinic in Madison, WV but will also work at other BMH locations, as needed.

The Advanced Practice Provider is expected to perform the following essential functions:

  • Provide primary care to patients in accordance with the BMH, Medical Staff, and RHC policies and procedures;
  • Comply with all appropriate requirements for clinic quality assurance and certification;
  • Maintain licensure, board certification, and current ACLS and PALS certifications;
  • Work with health care team to provide services to patients within the applicable standards of care;
  • Order laboratory and radiology tests and refer patients to specialists when appropriate and clinically indicated;
  • Analyze reports, test results, medical records, and examinations to diagnose condition(s) of patients and propose treatment options;
  • Prescribe pharmaceuticals, other medications, and treatment regimens to treat identified and documented medical conditions within scope of practice;
  • Discuss potential side effects of medication/treatment with patients and ensure that all allergy information is up-to-date before prescribing medication/treatment;
  • Provide advice to patients for lifestyle and diet changes that may improve their health or help treat the health issue they are experiencing;
  • Perform outpatient procedures which are within the scope of practice, education, supervision, and experience of the practitioner;
  • Adhere to departmental policies, procedures, and objectives; ongoing quality improvement objectives; and safety, environmental, and infection control standards;
  • Maintain patient confidentiality and comply with all federal and state health information privacy laws;
  • Ensure that current best practices are being implemented in order to ensure the highest standards of quality;
  • Complete timely and thorough patient documentation using the Electronic Health Record utilized by BMH, as directed;
  • Participate in Medical Staff and Quality Assurance committee meetings; and
  • Assist the Outpatient Clinical Director and Medical Director in development of new policies and procedures, as needed.
Skills, Qualifications, and Competencies Required
  • Nurse Practitioner or Physician Assistant competencies, as applicable (available upon request).
  • Incorporates professional/legal standards into practice.
  • Acts ethically to meet the needs of patients.
  • Assumes accountability for practice and strives to attain the highest standards of patient care and safety.
  • Engages in self-evaluation concerning practice and uses evaluation information, including peer review, to improve care and practice.
  • Collaborates and/or consults with members of the health care team about variations in health outcomes.
  • Uses an evidence-based approach to patient management that critically evaluates and applies research findings pertinent to patient care management and outcomes.
  • Evaluates the patient's response to the health care provided and the effectiveness of the care.
  • Uses the outcome of care to revise care delivery strategies and improve the quality of care.
  • Accepts personal responsibility for professional development and the maintenance of professional competence and credentials.
  • Considers ethical implications of scientific advances and practices accordingly.

Physical demands
  • Ability to perform various motor skills such as; standing for long periods of time, walking, lifting (up to 50 pounds) using proper lifting techniques, stair climbing, reaching, bending, twisting, pushing, pulling, squatting, grasping and arm-hand coordination on a continuous basis.
Travel required
  • Up to 10% of time may be spent traveling within the State of West Virginia and surrounding areas.
Required education, licensure, and experience
  • Active license issued by the West Virginia Registered Nurses Board to practice as an advanced practice registered nurse or active license issued by the West Virginia Board of Medicine or West Virginia Board of Osteopathic Medicine to practice as a physician assistant
  • 1 or more years of experience practicing as an advanced practice provider
